This form enables parents/carers to notify the school of a student's departure and provide important information regarding their final date of enrolment, reason for leaving, and intended future education or pathway. Providing accurate and timely information helps ensure student records are updated correctly and supports a smooth transition to their next educational setting or work pathway.
